Initial Consultation & Evaluation
Prior to an evaluation, we will schedule a free 10-15 minute phone call to determine whether my practice may be a good fit for your needs. If so, we will meet in person for a thorough evaluation to establish care and delineate a treatment plan. For adults, the evaluation usually comprises a single 60 minute session. For children and adolescents, the initial evaluation includes a 60 minute session with the adolescent or parent, a 45 minute session with the child or parent and a 15 minute phone session with the school. For family-based treatment for eating disorders, the initial evaluation is 90 minutes with the parents and child. The second visit will be a family meal and is scheduled for up to 90 minutes.

Fees
I am out-of-network with all insurance providers; however, I can provide a bill that you can give to your insurance company to request reimbursement. Please contact your insurance company directly to inquire about your out-of-network benefits. I am happy to provide a Good Faith Estimate of expenses as required by the No Surprises Act anytime at your request. All fees will be subject to an annual 2.5% increase on January 1st to adjust for inflation.

Forensic Evaluations
Please alert me prior to scheduling if you are interested in a forensic evaluation; my fee schedule and evaluation process is specific to the evaluation requested. Please note: I do not provide fitness-for-duty, workers' compensation or disability evaluations.

Scheduling
Initial evaluations and weekly recurring treatments will be scheduled directly with me over the phone. Appointments can be scheduled at least two days in advance and not more than 60 days in advance.

Cancellations
I require all patients to keep a credit card on-file. All sessions will be billed prior to the session. No shows, cancellations or rebooking with less than 48 hours' notice will be charged in full.

Controlled Substances
Due to limitations of my practice, I do not provide treatment to adults requiring regular prescription of controlled substances. I do provide full-range evaluation and treatment for children and adolescents, including with controlled substances when indicated.

Prescriptions, Care Coordination and Phone Calls
If you would like to use insurance for prescription medications, please obtain a copy of their price list / formulary. I bill per hour in 15 minute increments (not rounded) for all work outside of appointments and documentation, including prior authorizations for medications, school conferences, and clinical phone calls. The following services, however, are not billed: brief, non-urgent phone calls/portal messages regarding scheduling or receiving of documents and professional coordination of care with other physicians, therapists and school personnel.

Lab Work
Bloodwork and/or urinalysis are often indicated as part of an initial evaluation or ongoing treatment. I will order tests through national lab companies. If your insurance company requires that your primary care (or another in-network) physician orders these tests for reimbursement, I will happily coordinate with them.

Emergencies
I do not provide 24/7 emergency coverage. In the event of an emergency, please call 911 or go to the nearest emergency room. You, or the emergency room physician, may call me once you are in a safe place to further coordinate care.
